Pediatric to Adult Rheumatology Care Transition
The ACR has created juvenile idiopathic arthritis (JIA) and lupus toolkits to assist in the transition of young adults phasing out of a pediatric rheumatologists practice to an adult rheumatologist. The toolkits are split into two groups: Pediatric Rheumatology and Adult Rheumatology.See a brief explanation of the tools in the ACR Transition Toolkit.

About the Toolkit
The ACR Pediatric Transition Toolkit was designed as part of a broader initiative led by the American College of Physicians (ACP) Council of Subspecialty Societies in partnership with GotTransition. The broad ACP workgroup composed of CSS members as well as outside organizations developed common tools/templates for a “general” transition process, and subspecialty societies utilized the templates to develop customized toolkits for specific diseases/conditions.
