About the Toolkit

The ACR Pediatric Transition Toolkit was designed as part of a broader initiative led by the American College of Physicians (ACP) Council of Subspecialty Societies in partnership with GotTransition. The broad ACP workgroup composed of CSS members as well as outside organizations developed common tools/templates for a “general” transition process, and subspecialty societies utilized the templates to develop customized toolkits for specific diseases/conditions.

Guideline and Criteria App

The ACR’s Guideline and Criteria App is the go-to mobile tool for clinicians to implement guideline recommendations and criteria sets into practice. The app features interactive tools to inform treatment decisions at the point of care, including algorithms, scores and calculators, full guideline and criteria content, keyword searches, customizable bookmarks, and note-taking capabilities.
